Witnesses said the attack happened after an argument developed between the man and the woman in the town of Reutlingen, near Stuttgart.
The incident comes amid tension after other bloody events in the past week. A shooting rampage in Munich left nine dead while an axe attack on a train a week ago injured several people.
A 21-year-old man has been arrested over the attack near the bus station in Reutlingen.
Police said he was acting alone and there was no likely danger to other people now.
They also said he had been involved in previous incidents where he had caused injury, but gave no further details.
